Submission of this requisition form does not guarantee admission.
Contact numbers/E-mail IDs given must be permanent, valid and working. In the eventuality that the school is unable to reach you at the number(s) given, the school will not be held responsible for the consequences.
To fill up the online requisition form (over the internet) you will need either Firefox browser (ver 30 or higher) from Mozilla or Google's Chrome browser. Microsoft Explorer ver 11 onwards will also work.
Complete the form in one sitting. After you fill up the online requisition and "SUBMIT" it, you will receive an acknowledgement on your e-mail
In case you don't receive the acknowledgement mail in your INBOX, check your SPAM box also or contact school ( school email id)
If the Child is shortlisted, at the time of verification the following original documents to be submitted as given in the requisition form

Date Of Birth Certificate
Five Passport size Photographs of child.
Photographs of father and mother (one each).
Residence Address Proof
(Aadhaar Card / Passport/ Ration Card / Voter Id of parents)
Community Certificate for those other than OC.
Aadhaar Card of the child.

If the documents submitted at the time of the verification do not match with the entries in the requisition submitted online, the requisition and any further processing of the requisition based on the information submitted online will stand Null and Void.
